I'm trying to figure out the best way to install a ceiling fan beneath my pyramid-shaped roof. Specifically, what would be the best approach to ensure the fan is properly positioned and squared off?

Not totally sure what you mean by 'squared off', but they do make wedge mounts designed for hanging fans at the peak. Just a heads up, though, if your roof has four slopes instead of two, a cathedral mount might not work. You might need to build a custom box instead.
You don't actually need to square anything off. A ceiling fan extension rod kit is a solid option—just lower the fan enough so that the blades can clear the sloped ceiling. Makes installation a lot simpler!
That sounds good! But how do I securely attach the extension rod to the peak?
