I've got a towel holder that's mounted on the wall using toggle bolts, but no matter what I do, it keeps falling off! I make sure to tighten the set screw as far as it can go, and at first, it seems secure. However, after a short period, it just drops down again. What could I be missing here?

Are you sure the towel holder is actually falling off the wall itself, or is just the bar coming off the mounts? If it's just the bar, you might want to try using some Loctite on the set screw, specifically the low-strength kind like 222. It can help keep it secure without making it a pain to remove later.
If it's the mounts coming loose from the wall, you might want to consider relocating the towel holder to a more stable area. It sounds like the current spot could be weak. Finding a new location a good distance away from the existing holes and using proper wall anchors might do the trick!
