I'm having trouble with my toilet tank – it just won't fill up, and I'm really confused about where the water is going. Any ideas on what might be causing this issue?

If you notice that water is running continuously but your tank isn't filling up, it might be because the flapper is stuck open. In that case, water keeps flowing into the bowl and then down the drain. You might want to check that first!
If you're looking for more insight, try showing us the inside of the tank. It could help pinpoint the issue better. Though, I get it can be a pain to post images in certain places!
