I received a plant with a messed up label from a seed company, and I'm trying to figure out what it is. It has gone to seed and I'm not sure if it's bok choy or something else. Can anyone tell me what this plant could be?

It resembles mustard flowers to me. If it looks like those, then you might be dealing with mustards!
I think it's some kind of brassica. Could be bok choy, but if that's the case, it went to seed really quickly! What are the temperatures like in your area right now?
It looks like your bok choy has bolted, which can happen if it's too hot. That's when it sends up flowers before you're ready to harvest. How hot is it where you are?
