I noticed that one of my bathrooms is missing a towel bar, which makes me think it might have been taken down and never replaced. Is there a rule or building code that states a towel bar stud must be present in every bathroom? In my other bathroom, it's mounted about 4 feet off the ground. Can I assume there's a stud located at that height?

There's actually no building code requiring a towel bar stud in bathrooms. While it's a good practice to have one for support, they're often not installed. When I remodeled my bathroom, I made sure to put one in, so my towel bar is super sturdy!
You won't find any specific code for in-wall blocking, except for grab bars in accessible bathrooms. For your towel bar, it's best to use screws that anchor into the drywall or make sure it aligns with a vertical stud for better support.
Keep in mind that studs are vertical, so you can mount your towel rack at any height that suits you! It doesn't have to stick to that 4-foot mark—just find the studs first.
