How about using a Bluetooth keyboard with multimedia buttons? Some keyboards have specific buttons for play/pause, next, and volume controls. You could create a small controller using a microcontroller that has Bluetooth HID capability. An ESP32 would work nicely for this, even though it might be overkill. You’d connect it to your phone, and pushing the button would resume or pause the audio without anyone noticing.
You might want to consider using hearing aids as a base idea since they're designed to be discreet and can function similarly. Alternatively, just go for regular bone conduction headphones. People usually won't bat an eye if you say they're for 'magnetic therapy' or something like that, especially if they don’t know what bone conductors are.
