I installed a tile shower three years ago and have been using a tension rod for the shower curtain. It worked perfectly for the first couple of years, but I've faced a lot of issues lately. The rod has fallen several times and even bent. I've tried buying new rods, using sticky holders for extra support, and even gorilla glue tape, but nothing seems to work; it all ends up falling. I recently just put the liner up, and guess what? It fell again! I'm really hesitant about drilling into the tile because I'm worried it might crack, especially since there are metal studs behind the cement board and red guard. Does anyone have any suggestions?

You might want to try using some kind of rubber material, like the kind used to open jars, between the wall and the rod. It could provide some grip. Is the rod tight enough against the wall?
If you're dealing with a defective rod, returning it for a new one could be a good first step. Sometimes the simplest solution is to just replace faulty products. Let me know how that goes!
Consider using some heavy-duty double sided tape—around 35 or 50 pounds. I've used it to hold up various items, and once it's stuck, it doesn’t move! That might keep your holders secured.

It's pretty tight. It's not perfectly square, but it's close to what I built, so that's probably not the issue.
