What’s Going Wrong with My Cabinet Hinge Installation?

I've been having constant trouble with my cabinet hinges. They keep falling off the frame every few months, and I've tried a bunch of solutions to strengthen the mounting area and existing holes. I've used toothpicks, golf tees, wooden dowels with glue or epoxy, and even experimented with different screw lengths and types, including pre-drilling and self-tapping screws. I'm getting concerned because I'm running out of mounting area to fix these issues. What am I missing?
