Why is my driveway crumbling and how can I fix it?

I've noticed my driveway crumbling after two winters, and I'm trying to figure out if it's due to freeze and thaw cycles or if there was a problem during installation. My house is about two years old, and I've attached a picture for reference. What can I do to remedy this issue?
